Texas insurance is its own beast — no Medicaid expansion, 254 counties of network variation, and a marketplace that changes every year. Here are honest answers to the questions our Texas clients ask first.

No. Texas is one of the few remaining states that did not adopt Medicaid expansion under the ACA. As a result, roughly four million Texans fall into the 'coverage gap' — earning too much to qualify for traditional Texas Medicaid but historically too little to qualify for marketplace subsidies. Enhanced premium tax credits have closed much of that gap in recent years, and many Texans now qualify for plans with very low or even zero monthly premiums after subsidies. A licensed Texas Health Plans advisor can confirm what you qualify for in your county.

Self-employed Texans — freelancers, oil & gas contractors, real-estate agents, hairstylists, tradespeople, farm and ranch operators — can enroll in ACA marketplace plans during Open Enrollment (November 1 – January 15) or after a qualifying life event. Plans are available in all 254 Texas counties from carriers including BCBS Texas, Ambetter, Molina, Oscar, and UnitedHealthcare. Many self-employed Texans also qualify for premium tax credits based on projected business income.

CHIP (the Children's Health Insurance Program) in Texas provides low-cost health coverage for children in families that earn too much to qualify for Medicaid but cannot afford private coverage. CHIP Texas covers regular checkups, immunizations, prescriptions, dental, vision, hospital care, and mental-health services for children under 19. Your Texas Health Plans advisor will check whether your kids qualify for CHIP while you're shopping for adult marketplace coverage — the two programs can be coordinated in one household.

The major carriers offering individual and family plans in Texas include Blue Cross Blue Shield of Texas (BCBS Texas) — the largest by enrollment — along with Ambetter (Superior HealthPlan), Molina Healthcare, Oscar Health, UnitedHealthcare, and Aetna CVS Health. The 'best' carrier depends entirely on your county, your providers, your prescriptions, and your budget. Networks and formularies vary significantly between Houston, Dallas, San Antonio, Austin, and the smaller markets — which is exactly why we compare them side-by-side rather than recommending one carrier across the board.

Yes. Texas farm and ranch workers — including seasonal agricultural workers in the Rio Grande Valley, the Panhandle, and East Texas — can enroll in ACA marketplace plans the same as any other Texas resident. Because farm income often varies seasonally, premium tax credits can be especially valuable. We can also help families coordinate CHIP for children and discuss short-term bridge plans for periods between work seasons. Bilingual advisors are available.

ACA premium tax credits are calculated against the Federal Poverty Level (FPL) and have been expanded under recent legislation. In Texas, individuals and families earning between roughly 100% and well above 400% FPL may qualify for premium subsidies. As a guideline, a Texas family of four can often qualify for some level of premium assistance up into the low-six-figure income range. Income, household size, and county all affect the actual credit amount — your advisor will run the exact numbers for your situation at no cost.

Open Enrollment for Texas marketplace plans runs November 1 through January 15. Outside that window you'll need a Qualifying Life Event (losing job coverage, marriage, birth, move, etc.) to trigger a Special Enrollment Period. To enroll, you can use Healthcare.gov directly, but most Texans choose to work with a licensed Texas advisor — at no cost — to compare carriers, confirm provider networks, check subsidy eligibility, and submit the application correctly.
